The contact phone number for Care Calls is collected on admission and populates in VCE from the ADT feed. The team making post discharge calls often finds that the phone number is not the correct number - it may be that of a family member or friend or just not the preferred contact number. There is not an option to either edit the primary contact number or add another number. Currently the team at Englewood is putting the preferred number in the notes of the survey they are completing. If a follow up call is needed, the incorrect number is displayed on the survey tool with no indication that it is not the best option. The user would have to locate and view the previous survey to obtain that information. This is a time consuming process and could result in multiple attemtps to call an incorrect number.
