BCH has both 5 digit and 10 digit internal extensions. This requires callers to dial "#" after entering the extension to connect the call. Server 4.11.12 and app 4.12.4 was released to address the 10 digit internal extensions but it did not solve the problem because there is no way to program a "#" at the end of the dial pattern.
